This single was released with two different sleeves: one by Wendy Smith (above) featuring an illustration of two dancers, and another by Stuart (below) with a disturbing illustration of a baby.  The latter sleeve was banned after a very small initial pressing; the artwork currently appears in the Embrace the Herd reissue booklet.
